A SAFE PLACE
A Safe Place provides
support services to survivors of domestic violence in Strafford and Rockingham
counties. We provide one on one support, 24 hour hotline, emergency
confidential shelter, legal and social service advocacy, volunteer program and
community education. All the services that we offer
are free and confidential.

Agency Director, Pam Haggarty! We are so excited and fortunate to have
here with us, and we look forward to the community getting to meet her.
Secondly,
we are now offering a 4th Support group in the Portsmouth area, and this is
the only group that offers child care for kids up to age 12. It will be an
evening group. All groups held in a confidential location every week for 1.5
hours. Being part of the group can be an important part of breaking the
isolation that has been imposed by an abusive partner. ASP has 4 sites – Portsmouth, Rochester, Salem and the shelter.

City Year New Hampshire's mission is
to support the positive, healthy development of children and youth through the
experiences of service learning and civic engagement, so that they grow up as
confident, compassionate, and active citizens. In partnership with middle and
high schools, community and faith based organizations, and civic agencies, we
provide innovative, out-of-school time activities that focus on community
service, leadership and teamwork skills, diversity awareness and appreciation,
and environmental stewardship. City Year also maintains an ongoing presence in
schools as role models, one-on-one mentors, and providers of support services
to each student body we serve. We currently have teams serving in Manchester,
Nashua, Seabrook, and Strafford County.
CROSS ROADS HOUSE
Cross
Roads House, Inc. is an Emergency and Transitional shelter for those facing
homelessness protection from exposure and hunger while providing opportunities
to move to independent and sustainable housing. Some of our services
include: adult GED preparation, mental health counseling, referral to substance
abuse treatment, access to primary care as well as job search and
vocational training. Also on site, we have AA/NA meetings 5 nights a week
and monthly testing from ARS.

EXETER HOSPITAL PROGRAMS and SERVICES
●HealthReach - Diabetes, Endocrine &
Nutrition Center, a department of Exeter Hospital, offers a variety of
services: individual and group consultations with certified diabetes educators
to help individuals of all ages deal with the diagnosis of diabetes, regulation
of blood sugars and planning nutritious meals; diabetes fitness classes held at
Synergy on the Exeter Hospital campus; support groups for those with type 1 and
type 2 diabetes; and community blood sugar screenings and educational
programs. Medicaid, Medicare and
financial assistance (through Exeter Hospital) are welcome. Healthcare provider referrals are necessary
for individual, group and fitness education. Appointments, classes and support

●The
Exeter Hospital Home Visit nurses offer a home visit within 2-3 days after being discharged from
the hospital. The purpose of our visit is to help families’ transition to the
care of the newborn at home & to the adjustment of parenthood. The nurses
assess & weigh the newborn, assist with any feeding difficulties &
review any infant care skills. A physical assessment is done on the mother
& newborn. The nurses will assess how mom is adjusting to her role. They
offer support & information on resources available at Exeter Hospital &
in the community. The new families are offered a choice of either the Exeter
Hospital Home Visit Program or other agencies in the area for a newborn home
visit. A Family Center Nurse will assist families in the hospital to schedule a
home visit before they are discharged.

improve the health of the community with the Mobile Medical Van, a 37-foot
health clinic on wheels. Working with

information to uninsured and underinsured children and adults in the greater

●Health Care for the
Homeless
In December 2002, Families
First became one of only two agencies in New Hampshire - and only 154 in the
nation -- to offer a federally funded Health Care for the Homeless program. A
mobile health team based at Families First visits shelters, soup kitchens,
churches and other sites where homeless people spend time, traveling on two
specially equipped vans provided by Exeter Hospital and AIDS Response Seacoast
to sites throughout Strafford and Rockingham counties. The teams provide health
care, substance abuse counseling and social work services. Twenty-five
nonprofit agencies partner
with Families First to
provide the program, which serves Strafford and Rockingham counties.

GUARDIANSHIP TRAINING PROJECT
Is sponsored by the
Community Council of Senior Citizens and the Elders Concern & Abuse
Prevention Task Force. Our focus is to
provide family members and community members seeking guardianship the
encouragement, support, guidance and training they need to be competent
guardians for their loved ones. We have
produced a video, "Considering Guardianship: The First Step", that is
available for sale ($18.95 per copy).
This fifteen minute video is a collage of real life experiences of
people in NH who have become guardians and makes an excellent addition to the
video library of any agency working with the elderly and incapacitated
adults. I am also available to go to
organizations in Rockingham and Strafford counties to view the video and
conduct an information session on the Guardianship Training Project.

HAMPTON CHILD AND
FAMILY PROGRAM
Hampton Child and Family Program provides preschool, extended
care, before and after kindergarten care and before and after Head Start.
We are the only childcare facility in Hampton offering scholarship
assistance to lower income working families in the Hampton, North Hampton,
Seabrook area. We also offer literacy and family programs. We
provide much of our assistance to working families that usually fall through
the cracks in society. They earn too much for state or federal
assistance, yet don't earn enough to pay for rent, utilities, and childcare for
their families. We interact with Hampton Community Coalition, Families
First, Greater Seacoast Interfaith Hospitality Network, and Head Start to
ensure all children in the Hampton area have equal opportunities.

Law project for Seniors., Senior Advise line, Issues: HICBIC; plan delays, denials of services, nursing home beds in
Rockingham County.
3. Other issues: Medicaid Block grants and what that will do to the fragile health
care structure for the uninsured and underinsured, Katie Becket program cutting
from 1200 participants to 200 clients.

Our mission is to assure
that all unsheltered homeless persons gain access to appropriate shelter and
other necessary services. Population
served includes street homeless and "hidden homeless," such as those
facing eviction, doubled-up families, couch surfers, people sleeping in
vehicles. This is done through visits
to homeless sites in wooded areas, abandoned buildings, under bridges, and
through referrals from non-profit agencies, hospitals, schools, towns, and
concerned citizens. Referrals are also
received from the NH Helpline @ 800-852-3388. The outreach worker networks with shelters statewide, and can
arrange transportation when necessary.

Medication Bridge
What is the Medication Bridge Program?
Medication Bridge is a program to help eligible uninsured
and underinsured patients of all ages to receive needed prescription
medications. Every major pharmaceutical
company has a patient assistance program that provides free prescription
medications to eligible patients.
Medication Bridge will work with your provider to apply for these
medications.
Who
is eligible?
●The
patient must be a US citizen.
●The
patient must not have any outside assistance in paying for their medications.
This includes Medicaid, VA, private insurance, etc.
●The
monthly household income must be less than 200% of Federal Poverty Level.
(Approx. $1400/month for household of 1 and $1800/month for household of
2)
●The
patient must be taking a long-term medication.
●The
patient must be a patient of a physician associated with Exeter Health
Resources.

You will be asked to provide a list of your current medications as well as
proof of income.

SEACOAST
HOSPICE
is
a non-profit agency dedicated to promoting quality of life for the terminally
ill and supporting family and friends throughout the illness and bereavement
process. Seacoast Hospice offers a
specially trained, caring team comprised of physicians, nurses, nursing
assistants, social workers, volunteers, bereavement counselors and spiritual
care coordinators who provide medical, practical and emotional care and support
to terminally-ill patients and their families.
Seacoast Hospice services are available for patients in the home,
nursing home, assisted living residence or hospital residing in Rockingham or
Strafford counties. Bereavement
Services are an important part of Hospice care and are also offered to the
community at large. Grief Support Groups
are open to anyone grieving a death and the Bridges program is designed for
children and young people ages 4-18.

●Transitions
Transitions is a community outreach program offered by Seacoast Hospice
to assist patients and their families as they transition through the difficult
events that accompany a prognosis of a life-limiting illness, from curative
treatment to end-of-life care. This non-medical program provides emotional
support and practical assistance through volunteer support and case management
services. Transitions is available for individuals who choose to pursue
curative treatment or palliative care (comfort-only treatment), and for those
who don’t want, or don’t qualify for, hospice care. Services are provided in
both Rockingham and Strafford County at no charge Elaine S. Wiesman,

SASS is a non-profit agency
that is dedicated to supporting victims/survivors in their efforts to heal from
the trauma of sexual assault and childhood sexual abuse, while striving to
prevent the occurrence of sexual violence in local communities and in society

THE STEP
BY STEP CAREER GUIDANCE PROGRAM
is a statewide job readiness activity for the New Hampshire
Employment Program. The NH Division of Family Assistance has contracted with
Second Start, an adult education center in Concord, NH, to administer the
program. Step by Step has fourteen offices and several appointment sites
located through-out the state. Step by Step
works solely with individuals receiving public assistance from TANF (Temporary
Assistance to Needy Families). Step by
Step's primary goal is to guide participants off of welfare and into a life of
self- sufficiency. Career exploration, Job Readiness, job search. They have people with skills ready to go and be
employed!
